The directory provides information about various older adult and caregiver services provided within the City, including the locations and telephone numbers of senior centers and community services. It outlines many other excellent programs offered to older adults and provides a list of caregiver resources in addition to emergency help contact phone numbers.

In-Home Assistance

Designed to assist frail older adults in maintaining an independent living by assisting with household tasks.

Nutrition Programs

Congregate Meals: We have multiple dining centers throughout the City that provide a hot nutritious meal five days a week. The meals are available to any senior. Suggested voluntary contributions are encouraged but are not required.

Home-Delivered Meals: Delivers one hot, nutritious meal to home-bound seniors five days a week.

Transportation

Door-to-Door: A curb-to-curb transportation service for frail older adults provided through one of our Multipurpose Senior Centers.

Wellness Education & Screening

Health services and medical screenings are provided at our Multipurpose Senior Centers.

Evidence Based Programs

Evidence-based programs are the result of a growing body of research in the social and behavioral
sciences demonstrating that certain approaches and strategies for working with youth, older adults and families can positively impact important problems such as energy, motivation, depression, falls, and management of chronic health conditions. Los Angeles has embraced these state-of-the-art approaches to stay healthy and implement programs citywide (specific program offerings vary by area).


CDSMP- Healthier Living / Tomando Control de su Salud
‍Chronic Disease Self-Management Program (CDSMP)
In a program developed for people with chronic health conditions, participants learn practical skills for managing their chronic health problems such as tools to communicate effectively with family, friends, and health professionals, problem-solving and relaxation techniques which result in improved health status and proper utilization of the health care system.


Tomando Control de su Salud
The Spanish version of the Chronic Disease Self-Management Program, designed in Spanish for Spanish speakers. Though similar to the English version, it was developed to be culturally appropriate.  Participants learn to self-manage their chronic condition through similar tools as the English version to obtain the same health outcomes.


Physical Activity / Fall Prevention
‍Active Start
Combines Active Living Every Day – a behavior change discussion class, and ExerStart – a beginning-level exercise class to help introduce physical activity into the lives of seniors.


A Matter of Balance
A program for people who have fallen in the past, who have a risk of falling, and who may be reducing their activities due to a fear of falling. Participants learn to view falls and the fear of falling as controllable; set realistic goals to increase activity, change their environment to reduce fall risk factors, and exercise to increase strength and balance.


Arthritis Foundation Exercise Program
Developed specifically for people with arthritis but beneficial to those without, this exercise program uses gentle activities to help increase joint flexibility and range of motion and maintain muscle strength. Different classes are available to fit each individual’s fitness level ~ with exercises done while sitting, standing, or on the floor.



Arthritis Foundation Walk with Ease Program
A program developed to help individuals create and implement a walking plan to meet their particular needs. This program will help participants stay motivated, manage their pain, and learn to exercise safely to keep muscles strong and joints stable. Walk with Ease was created to help boost participant energy, reduce stress, and control weight.



Caregiving /Memory
‍UCLA Memory Training
Memory Training course provides an innovative educational program for people with age-related memory concerns. Participants learn in small groups, through a combination of presentations with group discussions, memory quizzes, and skill-building exercises. This program focuses on teaching new skills in a low-stress, fun environment.



Powerful Tools for Caregivers
A workshop for caregivers to develop self-care tools to help them manage and reduce stress, communicate their needs to family members and healthcare providers, deal with difficult emotions, and make tough caregiving decisions. This workshop has been shown to significantly improve the caregiver’s emotions, including reduced anger, guilt, and depression.


Savvy Caregiver
A workshop intended to train families and others for the unfamiliar role they face as Caregivers for a relative or friend with Alzheimer’s disease or another form of dementia. The program focuses on helping caregivers think about their situation objectively and providing them with the knowledge, skills, and attitudes needed to manage stress and effectively carry out the caregiving role.

Social & Recreational Activities

All our Multipurpose Senior Centers offer an array of activities and programs designed to enrich the lives of older adults. Activities may range from exercise classes to educational resources about healthy living and even day trips to local favorites.

Case Management

Designed to assess the social and health needs of frail older adults, develop a care plan to meet these needs, and ensure that the care plan works as designed.

Emergency Alert Response System (EARS)

The Emergency Alert Response System (EARS) is a medical communications and personal response system ensuring a quick and efficient response to emergencies in the home. Assistance is available 24 hours a day, 7 days a week, year-round. In addition, the program provides social reassurance to relieve isolation and improve the quality of life.

Requirements:

• 62 years of age or older
• Medically frail and/or home bound
• Must live alone or with a severely disabled person
• Reside in the City of Los Angeles
• Meet income eligibility guidelines and be on a fixed-income
• Must be enrolled in Case Management services through the local Multipurpose Senior Center

How Does it Work?

When a button is pressed on the unit or pendant, the 24-hour emergency response center is contacted. Within seconds, there is direct voice communication with an operator through the built-in speaker/microphone, at which time the user can communicate with the operator. In the event of an emergency, help is immediately dispatched, and important medical information is relayed to the paramedics and emergency room personnel to ensure appropriate and timely treatment. In addition, family members or the listed emergency contacts are notified.

How Much Does it Cost?

This service is provided at no cost to qualified older adults. Based on our requirements, those who don’t qualify may pay the unsubsidized costs directly to the service provider.
